• Observed raw chicken and beef stored above multiple ready to eat deli meats.
• Found multiple ready to eat foods in multiple coolers between 48F and 56F. Shall be 41F or less.
• Observed a gap in the back door of the establishment.
• Observed multiple rusty dry storage shelves.
• Found multiple broken cooler gaskets on multiple coolers throughout the establishment. Observed two broken hood covers on the reach in coolers.
• Observed accumulated old food debris around the meat cooler. Found black and blue like substance in the bottom of a reach in cooler on cook line.
• Observed black accumulated debris on the soda nozzles in the dining area.
• Observed accumulated debris on floors, walls, dish area, and in and around equipment throughout establishment.

• Observed employee drink and phone stored on prep surfaces.
• Observed cooked hamburger patties left in microwave from prior day.
• Observed accumulated debris on table mounted can opener and cappuccino machine nozzles.
• Observed accumulated debris on cooler shelves and door tracks throughout establishment, and on steam table.
• Observed accumulated grease on ventilation hood.

Applebee’s, 2506 S. 3^rd St. (4 Non-Compliance)

• Observed employee drink and phone stored on prep surfaces.

• Observed pot roast in walk-in cooler at 81F after 3 hours of cooling. Shall be 70F within 2 hours.
• Observed multiple wiping cloths resting on a variety of surfaces throughout establishment.
• Observed clean food containers stacked while wet.
• Observed broken floor tiles and low grout.

Comfort Suites, 501 Margaret Ave. (3 Non-Compliance)

• Found bag of sausage patties sitting in the hand wash sink.
• Found chemical spray bottle in kitchen area without a label.
• Found frozen sausage patties thawing at room temperature.

Days Inn & Suites Terre Haute, 101 E. Margaret Dr. (3 Non-Compliance)

• Found hand wash sink with no soap and no paper towels.
• Found reach in cooler without thermometer.
• Observed dead roaches, egg casings and mouse droppings in cabinets.

Twiggy’s Sweet Lou’s, 8567 E. Wabash Ave. (3 Non-Compliance)

• Observed accumulated carbon and food debris on pizza screens.
• Observed accumulated rust on walk-in cooler shelves, inside surface of door and on fan guards. Found kitchen walls chipped and worn down to bare wood.
• Observed accumulated debris on walls, floor and equipment.

Bev’s Country Kitchen, 1303 S. 25^th St. (2 Non-Compliance)

• Found meat loaf, lasagna and Swiss steak gravy between 47F-49F in the back of house cooler.
• Observed raw shelled eggs stored above multiple ready to eat foods.

• Observed ice and straws in bar hand wash sink. Shall be used for hand washing only.

• Observed gap at bottom of exterior back door.

• Observed accumulated debris on shelves in dairy cooler.

• Observed accumulated debris on reach-in cooler and freezer shelves.

• Observed multiple grab-n-go sandwiches without dates of consumption.

• Observed gap at bottom of exterior back door.

• Observed low grouted areas throughout the cook line, drive-thru area and in the dish area. Found rusty shelving in walk-in cooler.

• Found no sanitizer test strips to test sanitizer concentration.
