La Isla Mexican Restaurant, 1480 Ft. Harrison Rd. (6 Non-Compliance):
- Found cooked beef stored under raw beef and chicken on cook line.
- Observed spicy chicken not being cooled within proper time parameters.
- Found Chorizo, tilapia and corn dogs between 45-47F. Shall be 41F or lower.
- Observed multiple soiled/wet wiping cloths on multiple food prep surfaces.
- Observed rust on inside part of walk-in cooler door. Found foil, lining cart at the end of the cook line. Observed cardboard lining stock room shelves in back.
- Found toppings cooler hood broken off the hinges on the cook line. Found broken cooler gasket in server’s station cooler.
Subway, 1735 N. 3rd St. (5 Non-Compliance):
- Observed food debris in basin of hand wash sink.
- Observed accumulated pink and black debris inside ice machine.
- Found meatballs not using proper methods to reheat to required temperature within required time limits.
- Found meatballs being held at 94F.
- Found ready to eat foods without required date markings.
Infinity Petroleum, 5083 N. Lafayette Ave. (4 Non-Compliance):
- Found back room hand wash sink without paper towels.
- Observed accumulated debris in ice machine.
- Found wall around mop sink damaged and crumbling.
- Found establishment without sanitizer test strips.
Sunrise & Dine, 5035 N. Lafayette Ave. (2 Non-Compliance):
- Found a bowl used to scramble eggs stored on top of a pan containing sliced tomatoes.
- Observed sanitizer in dishwasher not measuring at a proper concentration to disinfect.
Dairy Queen, 3201 Wabash Ave. (1 Non-Compliance)
- Observed accumulated debris on walls and floor drain.
