Observed chipping and peeling wall material behind 3-bay sink and in walk-in cooler. Observed bulk ingredients and spices not labeled.
Taj-Mahal, 1349 S. 3rd St. (7 Non-Compliance) Found sauce/gravy held at 100F. Shall be 135F or above. Found various chutneys and ready to eat items without date markings. Found raw chicken stored over fresh produce. Found frozen eggplant thawed improperly at room temperature. Observed accumulated debris on shelving and on fan guards in walk-in cooler. Observed gap at bottom of exterior kitchen door. Found dessert items in grab and go cooler not labeled with ingredients.
Top First Wok, 2570 Wabash Ave. (6 Non-Compliance) Observed gap at exterior kitchen door. Observed excessive accumulated debris on floors in dish area and under 3-bay sink. Observed excessive accumulated debris on handles of reach in coolers and inside of reach in freezers. Observed accumulated rust on food storage shelving and broken and cracked lid on reach in chest freezer. Found bags of onions and other food items stored directly on the floor. Found raw chicken stored above ready to eat food items.
Luca Pizza Di Roma, 3401 S. US Hwy 41 (5 Non-Compliance) Observed accumulated debris in ice machine. Found cooked meatballs and pasta in cooler without date markings. Observed cooked pizza and other cooked items held at room temperature without time records. Found a bare wooden shelf, broken floor tiles and low grout. Observed accumulated debris on floor, walls, storage shelves and equipment.
Greek’s Pizzeria, 600 Wabash Ave. Suite B (5 Non-Compliance) Observed accumulated carbon buildup on multiple pizza screens in kitchen. Found accumulated debris in ice machine. Found pre-rinse spray nozzle in ware washing areas encrusted in old food debris. Observed accumulated debris on the semolina container. Observed accumulated debris on floor, and in and around equipment throughout kitchen. Found employee drinks and personal items in the walk in cooler and on prep surfaces. Shall be in a designated area. Observed accumulated dust and debris on ventilation hood above pizza oven.
Outback Steakhouse, 3700 S. US Hwy 41 (5 Non-Compliance) Observed raw chicken breast in cooler at 47F-57F. Shall be held at 41F or below. Found gap under exterior kitchen door. Observed low grout and broken floor tiles. Observed torn gaskets on cooler drawers, broken and hanging ceiling vent. Observed accumulated debris on floors, on, under, and around equipment.
Sonka’s Irish Pub, 1366 Wabash Ave. (4 Non-Compliance) Observed refrigerator shelf and drawers broken. Found walk-in freezer door gasket broken and torn. Observed accumulated ice and frost inside walk-in freezer. Observed accumulated rust on storage racks and bare wood on walk-in cooler shelves and throughout kitchen. Observed accumulated debris on walk-in cooler fan guards.
Maurizio’s Pizza, 2940 Wabash Ave. (3 Non-Compliance) Found accumulated debris on can opener blade. Observed rust on multiple shelves in walk-in cooler. Observed accumulated debris on the outside of the dough mixer and cheese warmer.
Stables Steakhouse, 939 Poplar St. (3 Non-Compliance) Found gaps in multiple back doors in kitchen. Found raw wood and rust on multiple surfaces throughout kitchen. Observed a build-up of debris on pre-rinse spray nozzle in ware washing area. Found accumulated debris on oven doors throughout kitchen.
Sycamore Manor, 222 S. 25th St. (3 Non-Compliance) Observed employee drink without lid stored on cart near service line. Observed multiple torn and broken reach in cooler door gaskets. Observed holes and peeling wall material around ware washing machine, 3-bay sink and prep sink.
Dairy Queen, 1331 N. 13th St. (1 Non-Compliance) Observed missing grout in between floor tiles near food prep station and chipping and broken front counter piece near window.
