- Found chicken improperly cooled.
- Found multiple foods stored in coolers between 48F-52F. Shall be 41F or lower.

- Observed multiple foods thawing improperly throughout kitchen.
- Observed rusty cooler shelves in multiple refrigeration units.

- Observed multiple employee drinks without lids and improperly stored.
- Observed large spoon resting in hand wash sink.
- Observed shredded chicken and queso made the day prior, reheating on steam table at 74F-86F.
- Observed cockroach on wall in dining room.
- Observed clean cups stacked while wet.
- Observed peeling wall material, bare dry wall, rusty storage racks and broken door trim on walk-in cooler.
Walmart #1310, 5555 S. US Hwy 41 (4 Non-Compliance)
- Observed accumulated debris on bakery pans and rotisserie racks.
- Observed accumulated debris on deli 3-bay sink spray nozzle, on multiple walk-in cooler fan guards, on reach-in cooler and freezer shelves, on dairy walk-in cooler shelves, on floor under eggs and on cooler door track in deli area.
- Found rusty storage racks in dairy and produce walk-in cooler and rusty rolling carts. Found chipped floor and door trim paint in deli.
- Found several walk-in freezers with accumulated ice and condensation on glass freezer doors.
Texas Roadhouse, 201 E. Turner St. (3 Non-Compliance)
- Observed accumulated debris on dish machine spray nozzle, salad plate cart, meat room, fan guards, in ovens and plate chiller cooler.
- Observed accumulated grease and debris on hot prep ventilation hood.
Vigo County Jail, 500 W. Honey Creek Dr. (3 Non-Compliance)
- Found taco meat being cold held at 47F. Shall be 41F or less.
- Observed accumulated debris on walk-in cooler fan guards.
- Found freezer jackets stored with kitchen equipment.
