Observed broken, soiled and ripped gaskets on doors of reach-in prep coolers. Found scoops without handles stored in sauces in walk-in cooler. Observed gap at bottom of exterior back door. Found buckets of raw chicken, various sauces and onions stored directly on floor of walk-in cooler.
Kroger’s, 2140 Ft. Harrison (7 Non-Compliance) Found raw pork and chicken packed in same bags with ready-to-eat food in grocery pick up refrigerators. Found accumulated food debris in tub at chicken breading station, on meat slicers in deli and on table mounted can opener in deli. Equipment not being cleaned with appropriate frequency. Observed accumulated debris on dairy shelves and on shelving with pre-packaged lunchmeats, refrigerated juices and eggs. Found debris on fan guards in dairy walk-in. Observed hand wash sink in deli area blocked and not accessible. Observed food and food items stored directly on floor. Observed excessive flies on slicers in deli area. Observed accumulated debris on floors, walls, and under and around equipment in deli area.
Royal Mandarin Express, 11 Meadows Shopping Center (7 Non-Compliance) Found employee drink sitting on toppings cooler next to various meats. Observed multiple employees cutting and mixing ready-to-eat foods without gloves. Found multiple foods throughout establishment being cold held between 48F-138F. Shall be 41F or less. Observed multiple foods throughout establishment without dates of consumption. Found multiple bulk ingredients not in their original packaging without their common name. Observed multiple foods being stored in cans. Shall be stored in a food grade container. Found pipe in ceiling dripping/leaking.
Wings Etc., 4680 S. US Hwy 41 (6 Non-Compliance) Found sanitizer at ware washing machine not measuring at proper concentration to disinfect. Found mac n cheese on steam table at 125F. Shall be 135F or more. Observed multiple bulk ingredients not in their original container without common name. Found accumulated debris on multiple soda nozzles. Observed accumulated debris on ice machine in kitchen. Observed broken hood for toppings cooler and broken door gasket on freezer on cook line. Observed accumulated debris on fan guards in walk-in cooler.
Real Hacienda, 3728 Wabash Ave. (6 Non-Compliance) Observed fly strip hanging above clean dishes. Observed cook not changing gloves between contact with raw meat and ready-to-eat food. Observed accumulated debris in ice machine and on table mounted can opener. Found queso on steam table at 102F. Shall be 135F or above. Found clean cups stacked while wet. Observed accumulated debris in reach-in freezers, oven and bar cooler. Observed accumulated debris on floor walls, on, under, and around equipment.
Cajun Cafe, 3401 S. US Hwy 41 (5 Non-Compliance) Found employee drinks, foods and other personal items on a prep surface and other locations throughout the establishment. Found various foods being hot held at 108F to 132F. Shall be 135F or more. Observed multiple bulk spices/seasoning not labeled with common name of ingredient. Observed buckets and boxes containing: 328 sauces on floor in dry stock room. Observed accumulated debris on shelving in walk-in cooler.
Cracker Barrel, 429 E. Margaret Dr. (5 Non-Compliance) Found wiping cloth buckets with no measurable sanitizer for disinfection. Found heat sanitizing dish machine not reaching proper temperature for disinfecting. Found accumulated debris in reach-in coolers and freezers around ice machine door and on storage racks throughout kitchen. Found multiple clean dishes stacked while wet. Found excessive accumulated debris on ventilation hood above flattop on cook line.
New Day Cafe, 2919 S. 3rd St. (5 Non-Compliance) Observed cook place cheese slice on dirty wiping cloth prior to placing on burger patty. Found wiping cloth bucket without measurable sanitizer. Found exterior screen door with torn screen and gap around and underneath. Observed accumulated debris on pre-rinse spray nozzle, walk-in cooler and freezer fan guards and in microwave. Observed accumulated debris on floor, walls, storage racks on, under and around equipment.
Anthony Square Congregate Living, 400 College Ave. (4 Non-Compliance) Observed employee drinks without lids stored above food items. Observed bagged raw chicken and bagged broccoli stored inside same container in cooler. Observed ware washing machine sanitizer not measuring at proper concentration to disinfect. Observed improper thawing methods being used.
Rick’s Smokehouse, 3100 Wabash Ave. (4 Non-Compliance) Observed cooked pulled pork in warmer at 120F. Shall be held at 135F or above. Observed collard greens and potato salad in walk-in cooler held past date of consumption. Observed accumulated debris in reach-in coolers, freezer and pre-rinse spray nozzle. Observed accumulated debris on floor, walls, on, under, and around equipment and storage racks.

Zeng Sushi, 2309 S. 3rd St. (2 Non-Compliance) Observed employee stir cooked noodles in the cooling process by hand without gloves. Observed accumulated debris in reach-in freezer on outside surface of equipment and on storage racks and carts.
