Found scoops without handles being stored in sauces in walk-in cooler. Observed accumulated debris on floors, walls and in and around equipment throughout establishment. Observed accumulated debris on shelving in walk-in cooler. Found cooked chicken, cooked noodles, raw beef and all other items in prep cooler at temperatures between 55-60F and prep cooler not holding proper temperature. All items were moved and/or discarded as deemed necessary. Found hoisin sauce stored in open can with no date marking.

Observed various raw meats stored above ready-to-eat foods in the walk in cooler. Found multiple wet and soiled rags throughout kitchen on prep surfaces. Observed accumulated debris on the inside of the ice machines. Found multiple ready to eat foods not marked with date of consumption. Observed multiple broken/torn cooler gaskets throughout kitchen. Observed sanitizer in the dishwasher not measuring at a proper concentration. Observed accumulated debris on floors, walls, dish area, and in, and around equipment throughout kitchen.
Applebee’s Neighborhood Bar & Grill, 1950 Ft. Harrison Rd. (7 Non-Compliance) Observed multiple employee drinks with and without lids throughout kitchen. Dishwasher not meeting proper temperature for sanitizer. Dishwasher not sanitizing properly. Found multiple clean dishes stacked while wet. Observed little to no grout throughout kitchen floor tiles. Observed rusty shelving in walk-in cooler and freezer. Found accumulated debris on fan guards in walk in cooler. Observed black accumulated debris in reach in toppings cooler on cook line. Observed accumulated debris on floors, walls, dish area, and in and around equipment throughout kitchen.
Original Spaghetti Shop, 2446 Wabash Ave. (5 Non-Compliance) Found accumulated debris on table mounted can opener and encrusted carbon and debris on conveyor oven. Found accumulated debris on fan guards in walk-in cooler and gaskets on reach in pasta cooler. Observed accumulated debris on ventilation hood system above conveyor oven. Found gaskets on reach-in pasta cooler ripped and broken. Observed wet and soiled mop stored in mop bucket.
Burger King, 2575 St. Rd. 46 (4 Non-Compliance) Observed accumulated black debris inside ice machine. Observed gap at bottom of exterior kitchen door. Observed standing and leaking water in area near walk-in cooler door and around water heater. Observed accumulated grease and debris on floors, walls, and in, around, and under equipment throughout establishment.
Burger King, 2116 Lafayette Ave. (4 Non-Compliance) Found excessive accumulated grease and debris around fryer area. Found accumulated debris on ventilation system above fryer area. Found cracked and missing tile coving in kitchen area. Observed accumulated debris on floors, walls and in, around, and under equipment throughout establishment
Rally’s, 3175 Wabash Ave. (4 Non-Compliance) Observed accumulated debris on fan guards in walk-in cooler on tops of fryers and under 3-bay sink area. Observed low and missing grout on floor tiles, cracked and broken lid on ice machine, broken and missing trim and coving around exterior door and dish area, and rusting shelves in walk-in cooler. Observed gap at exterior door. Observed accumulated debris on floors, walls, and in, and around equipment throughout establishment.
The Noodle Nook, 1275 Lafayette Ave. (3 Non-Compliance) Observed a sanitizer bucket and other supplies in the hand washing sink making it inaccessible for employee use. Found Bao buns being hot held at 97F to 111F. Shall be 135F or more. Observed cut cabbage and birria sauce held past date of consumption.
Subway, 2399 SR 46 (3 Non-Compliance) Observed employee use cell phone and not change gloves prior to food handling. Found excessive ice accumulation and condensation in walk-in freezer. Found peeling coving at threshold of walk-in cooler and ripped gasket on bottom of walk-in freezer door.
Walmart, 2399 St. Rd. 46 (3 Non-Compliance) Found many dented cans throughout aisles in store. Found accumulated ice buildup in the ice cream freezer, bakery freezer and the deli freezer. Observed accumulated debris on fan guards in the dairy, produce, bakery and deli walk-in coolers.
Fox Grocery, 6869 N. Clinton St. (2 Non-Compliance) Observed wiping cloths in use for food spills and splatters not stored in sanitizer solution. Observed accumulated debris on wall behind prep table and around hand wash sink.
Scout’s Pizzeria/Verve, 677 Wabash Ave. (2 Non-Compliance) Found multiple foods not marked with date of consumption. Observed broken cooler gaskets on reach in cooler in kitchen.
